This form is used by high schools to officially ask for Perkins funding to cover a need (equipment, professional development, etc). Schools who have submitted their Local Planning document (found below) in advance receive priority consideration. Visit our How To page for details on how to successfully fill out this document.

How to use this form:
This form is used by high schools to provide the Consortium with an idea of how they'd like to use Perkins money for the upcoming fiscal/academic year. These forms are critical for us to be able to budget accordingly to ensure that we have enough funds to be able to support your schools and project initiatives.
NOTE: Schools that provide these form on or before April 15th for the upcoming fiscal/academic year will receive priority consideration when submitting their formal requests during the academic year. Forms not received by the due date will still be accepted but formal requests received by these schools may need to wait for reallocation dollars (typically received in January). MILEAGE REIMBURSEMENT
How to use this form:
Teachers who are eligible for reimbursement of mileage should submit this form as their "receipt" along with the Requesting Perkins Funds document (if that hasn't been submitted already).
SUBSTITUTE TEACHER REIMBURSEMENT
How to use this form:
If your high school doesn't have a report or document to show the amount paid for a substitute, we ask that you please use this form along with the Requesting Perkins Funds document (if that hasn't been submitted already).
