Requesting Perkins Funds

Requesting Perkins funding from the Mid-Minnesota Consortium is as easy as 1...2...3

All requests need to be submitted on the Request for Funds form. Forms are found here. Please download or save a copy first before completing it.

Forms need to be completed fully and should include a signature from your principal and a quote.

Once completed, submit it via email to the Mid-Minnesota Perkins Coordinator for review.
