Requesting Perkins Funds

This form is used by high schools & Ridgewater College CTE faculty to officially ask for Perkins funding to cover a need (equipment, professional development, etc). 

Teachers & instructors are required to complete this for each item they want to purchase. This form is then reviewed by the assigned Perkins coordinator to ensure it follows Perkins funding guidelines. High schools and college programs that have submitted their Survey of Needs document (also known as the Local Planning document) in advance receive priority consideration.

Please note: If a teacher/school/district/program submitted the Survey of Needs document, it does not mean your request is pre-approved. That document is for planning purposes when we write our yearly application and proposed budget. A request form (dated within the current fiscal/academic year) is still required in order to be approved to use Perkins funding. If a request form is not submitted and approved by the Consortium Coordinators or MDE/System Office, final receipts cannot be turned in for Perkins reimbursement at the end of the academic year.

High School Faculty

  • The form below is for teachers requesting equipment, professional development, curriculum, field trips, and/or technology.
  • Please include a quote for all reimbursable expenses (including registration, hotel, mileage, subpay, etc.).
  • Acceptable quotes include:
    • a quote prepared by a vendor/company selling the item
    • a screenshot & link to the item being requested
    • an email from the vendor/company (this is most common for things like bus costs and substitute teacher base pay).

High School Staff

  • The form below is for non-teacher roles requesting bussing (for IGNITE, EXPLORE, EPIC, career exploration) & Big Ideas Sim Lab Rental for their school/district.
  • Please include a quote for all reimbursable expenses.
  • Acceptable quotes include:
    • a quote prepared by a vendor/company selling the item
    • a screenshot & link to the item being requested
    • an email from the vendor/company (this is most common for things like bus costs and substitute teacher base pay).

Ridgewater College Faculty

  • Reminder: Ridgewater College faculty will also need to complete a Purchase Order (PO) requisition in Workday. Resources on how to complete that requisition are found on the Business Office page in the Employee Portal.
  • The forms linked below are read only. You must download or save a copy first. Typed forms are preferred vs. handwritten.
  • Quotes will need to be obtained and submitted with your funding request.
  • Ridgewater College Sharepoint site is where the PO form is located.

When requesting funding, a quote is always required, but sometimes the information is shared in discussion with district or high school staff. If your request includes mileage and substitute teacher pay, but your district does not have anything in writing for either of these expenses, please use the form below and include it with your request paperwork.

This form is used to provide the Perkins coordinators with an idea of how schools/programs would like to use Perkins money for the upcoming fiscal/academic year.  This forms is critical for us to be able to budget accordingly to ensure that we have enough funds to be able to support your school/program and your project initiatives. Below are some helpful links to help you with your planning:

Please note: As noted above, completing this form is to assist us with budgeting only. It is not a purchasing pre-approval form. Schools/programs that provide these forms on or before the deadline for the upcoming fiscal/academic year will receive priority consideration when submitting their formal requests during the academic year.  Forms not received by the due date will still be accepted but formal requests received by these schools/programs may need to wait for reallocation dollars (typically received in January).

Schools should not be limiting who has access to funds each year. Every eligible department is to be planning on how they may want to use Perkins funds for the upcoming year.

Externships & Stipends

(High Schools Only)

Teachers who wish to advance their skills and knowledge in their CTE areas, may be eligible for externships and receive a small stipend. Externships are only available when a teacher is off contract (nights, weekends, breaks). Teachers will need to complete the Externship Agreement, submit a time card, and complete a W-9 tax form (if not already on file). Externships are for a maximum of 16 hours at a rate of $35/hr.


Stipends may also be awarded to high school teachers who participate in Mid-Minnesota Consortium Created Professional Development while off contract. Stipends are a flat rate (usually $350) and mileage is also covered. Teachers will need to sign into the approved sessions and stay for the full agenda of activities to be eligible for the stipend. A mileage form will need to be completed (found above) as well as a W-9 form (if not already on file).

These professional development offerings may also count for CEU hours needed to Minnesota Teaching License requirements. A certificate will be made available to teachers upon completion. It is up to the teacher to research and identify what is/isn’t CEU eligible. Programs of Study

Programs of Study

Although Mid-Minnesota Perkins partnership has had primarily the same programs of study for the past 16 years, the order of significance and workforce need has changed in the past two years. The consortium ranks the order of priority of programs of study and funding based on the workforce needs of the region (Economic region 6E). Several different data sources were used to determine workforce need (DEED, Lightcast, Realtime Talent -SW region, Career Wise, Agri Centric, economic information for Hutchinson area and Willmar area).

It’s important to remember that geographic areas do not always match nor do Programs of Study clearly delineate the career pathway (agribusiness, for example most likely is included in accounting, retail, marketing, etc). Any Program of Study that is identified by the consortium must meet two of the three criteria as outlined in Perkins V legislation regarding careers in these areas: high pay, high skill, and/or in-demand.

Plant and Animal Systems, Veterinary Technology, Meat Cutting, Food processing, etc.

Teaching and Training in Early Childhood (daycare, Pre-K, K-3, etc)

Business & Management

Accounting, Bookkeeping, Tax prep, Marketing, Entrepreneurship, Social Media, etc.

Transportation

Auto Mechanics, Auto Body Repair, Small Engines, etc.

Digital Arts

Photography, Design, Computer Programming

Culinary

Culinary Arts (FCS)
