Requesting Perkins Funds

All requests need to be submitted on the Request for Funds form. Please download or save a copy first before completing it. - Complete
Forms need to be completed fully and should include a signature from your principal and a quote. Read 5 Tips for Requesting Perkins Funds for advice! - Submit
Once completed, submit it via email to the Mid-Minnesota Perkins Coordinator for review!

When submitting a request, high schools should not limit which programs are eligible for funds each year. Funds are open for all eligible programs/departments.
